New Patients

Your First Visit

Your initial appointment will most likely consist of a consultation explaining your diagnosis and treatment options. Our front desk staff is well trained and will discuss your options during your first phone call to our office. Occasionally, treatment can be done the same day as the consultation if a clear diagnosis has already been made by your dentist. However, a complex medical history or treatment plan will require an evaluation, further consultation and discussion with your referring dentist, and a second appointment in another day to provide the necessary treatment.

Please assist us by providing the following information at the time of your consultation:

• Your treatment referral slip and any X-rays if applicable (it would be best to mail us the x-rays prior to the appointment if time allows)
• An accurate list of medications you are presently taking
• If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
• Please register online by filling out the registration form, the patient Privacy form, and the medical history forms on this site.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Note: If you are nervous, oral sedation is a great option. No one experiences any discomfort in our office and we guarantee that you will experience no pain while in our chair, but many people have a past history of bad experiences in childhood and are generally anxious towards dental treatment. For such patients, we recommend premedication with an anti-anxiety medication like (Valium, or Halcion). Our patients often fall sleep while we are working on them even without premedication! Doctor Nasseh teaches all the modern endodontic and patient management principals and techniques at Harvard and Tufts Dental Schools; however, should you still choose to take a sedative medication, please mention that to our front desk staff during your initial phone call. You may require a consultation visit first before the appointment to get a prescription. You can also ask for a prescription from your referring dentist prior to the appointment. If that is the case, you need to fill out all the medical history and consent forms by printing them from this site, sign and date them the day prior to your visit and bring them with you to your appointment. If taking a sedative you may not drive to and from the office unassisted. You could take a taxi however. We can call one for you at the end of the procedure.
